The probability of success evaluated by critical analysis of the literature is an important step in practicing evidence-based dentistry. The prognostic data in regards to endodontic microsurgery attests to the fact that microsurgical success rates are far superior than traditional surgery. At the same time, within the surgical technique, each step of the procedure is important to ensure a successful outcome. This presentation will critically review the available literature to evaluate mitigating variables including the indication for retreatment before surgery, reversal of success over a longer observation period, intraoperative variables such as, use of magnification and illumination, periodontal involvement, root-end management techniques as well as potential reasons for failure and the expectations from resurgery.
